Requesting a Copy of Your Health Records

You have the right to request and receive a copy of your health care records. To request your records, please submit your request in writing — by email or mail — and include the following:

  • Full name

  • Date of birth

  • Phone number

  • Email or mailing address — where you'd like your records sent

  • Which records you're requesting — for example, a specific date range, treatment summaries, or your full clinical file

Requests can be sent to joannc@unboundhealingandcounseling.org or mailed to 910 Collier St. Suite 225, Fort Worth, Texas 76102.

To protect your confidentiality, I may ask you to verify your identity before releasing any records. If your request is missing information, it may take longer to process — so the more detail you can give me up front, the faster I can get this to you.

Administrative fees may apply to cover the cost of copying and processing your records; you'll be notified of any applicable fees before your request is fulfilled.

Filing a Consumer Complaint

Most concerns that come up in the course of therapy can be worked out between client and clinician. Sometimes, though, a situation calls for more. If that happens, you have the right to file a complaint, either with BHEC or with the Texas Attorney General's Consumer Protection Division, without needing to contact me first. In compliance with the State of Texas, here's the information to assist you in that process.
